Life Events and Changes

The following qualify as life events and changes:
  • Marriage
  • Birth or Adoption
  • Spouse Loses Coverage
  • Dependent Loses Coverage
  • Divorce
  • Retirement
  • Military Service

These events enable you to make new decisions about your benefits coverages. You have 30-days from the date one of these events occurred to make changes. You may enroll or change the following coverages at this time:
  • Health Insurance
  • Dental Insurance
  • Flexible Spending Account: First Time Enrolling
  • Flexible Spending Account: Changing Election Amount
Complete the life event and change form.

Depending on what life event you experience, you may also need to update the following:
  • Name (if changing)
  • W-4
  • Direct Deposit (if opening accounts)
  • Address
  • Employee Biographical Information
  • Change in Beneficiary Forms
