Application Checklist

Preparing to submit your IRB Application

A minimum of three weeks prior to the estimated start date for your study, submit completed forms, consent forms, and any additional documentation required for your particular protocol to the current chair of the IRB Committee ("IRB Reviewed Protocols") or to the appropriate department chair ("Chair Reviewed Protocols"). Your materials may be submitted electronically or mailed.

Site Permission (if applicable)
Contact an appropriate party to seek written permission to conduct research in a particular setting. Print this communication and include it in your IRB application file.

Consent Forms
This (these) documents explain the research study and ask for assent to participate. There are examples of several types of consent forms to help you in creating those necessary for your particular study. Be sure to include all elements required (see details on the Application for IRB Reviewed Protocols - item 11). Print copies of your consent forms to include in your IRB application file.

Survey Instrument or Questions (if applicable)
Prepare the survey instrument or questionnaire you plan to distribute and/or list the interview questions you plan to use and include copies of these in your IRB application file. (These items may undergo changes during the course of your research. Unless changes are substantial, the IRB does not need to review them again.)

Grants Received (if applicable)
If you have received any type of grant funding, include a copy of the documentation identifying the nature and amount of financial support you have received in your IRB application.

Complete NIH Web-Based Training Course
Complete the online training program “Protecting Human Research Participants” and print your certification of completion. Submit this certificate with your IRB application file.

Additional Forms

Protocol Modification Request Form
This is a form that is submitted to the IRB if there is a change to your study. If there are major changes to your topic or methodology, you will likely be required to submit a new IRB application file.
