Notice of charges for violation of academic integrity

The definition of academic integrity at Concordia is outlined in the Concordia College Catalog.
 
The disciplinary process for violations can be found in the Concordia College Handbook.
 
You will need to print three copies of this form:
       1 copy is to be filed in the Academic Affairs office
       1 copy is to be given to the student involved
       1 copy is kept by the faculty member filing the complaint
 
Type in the information and then print three copies. The information you type will not be saved anywhere on the computer.
